By signing my name below and checking yes, I am certifying I have read and agree to the following conditions.
VEHICLE:
I am agreeing that the indicated vehicle above is my personal/business vehicle.
I am the owner of this vehicle and once I give you my keys, I am held liable for any and all damages to this vehicle and or trailer.
I am agreeing and stating that my vehicle being shuttled is fully insured as required by law and the insurance coverage will provide coverage to Central Idaho River Shuttles and the contracted shuttle driver.
I give permission to Central Idaho River Shuttles and the contracted shuttle driver to drive my vehicle for the purposes of this shuttle.
Yes--I certify that I as the owner am responsible for a full tank of gas, checking my oil, and all other upkeep of my vehicle and/or trailer when dropped off.
Yes-- I have listed the problems with my vehicle as honestly and specifically as possible.
Yes--I have a full tank of gas or near full and/or have left some money to have it filled with gas and or oil, transmission fluid, etc.
Yes--I have also given the key to the lock on the trailer and checked the oil, liquids, tires, spares and bearings on my trailer and/or vehicle.
Yes--If I have not left a key and need to cut the lock on the trailer in order to repair it, I agree that I will not hold Central Idaho River Shuttles responsible for this nor charge them for the lock.
Yes--I will disable the alarm system or describe the alarm system in detail with instructions to Central Idaho River Shuttles so that they may be able to drive it.
Yes--I am responsible fully for the insurance coverage on this vehicle and I hereby agree to hold Central Idaho River Shuttles, and the drivers contracted out by Central Idaho River Shuttles harmless of any liability or physical damage to my vehicle/trailer or personal property.
Yes--I understand that the roads we drive are hard on vehicles and trailers and any electronic damage from this because of the road the shuttle company will not be held liable for. I understand that the road driven may be rough and poorly maintained. I will not hold Central Idaho River Shuttles or the drivers responsible in any way for the damage done to the ascribed vehicle and or trailer while in route or parked for the duration of the shuttle.
Yes--I understand the person driving my vehicle is a legal, licensed driver.
KEYS
Yes-- I am responsible for leaving the key. If you do not leave the key where you said and we cannot find it Central Idaho River Shuttles are not responsible for not being able to deliver the vehicle. We need you to leave the key.
INSURANCE
Yes--Permission is hereby granted to Central Idaho River Shuttles and their contracted driver to drive the vehicle above and the vehicle is covered by Bodily Injury and Property damage Liability Insurance, and Physical Damage for vehicle and trailer. The insurance will provide coverage for the vehicle, the contracted shuttle driver, and Central Idaho River Shuttles. The vehicle and or trailer has at least $100 deductible collision and comprehensive insurance. The vehicle owner/operator hereby covenants and warrants that the vehicle has full collision insurance and liability insurance in the minimum amount required by the laws of the State of Idaho.
Yes--If the insurance does not provide the above, I understand that I will be held liable and responsible for charges for damages resulting from any deficiency in insurance coverage. The vehicle owner/operator shall forever indemnify and hold harmless Central Idaho River Shuttles and the persons operating the vehicle for shuttle service against any and all loss, cost, claim or damage an account or any injury or damage to persons or property arising out of the performance of this contract. The vehicle owner/operator acknowledges this risk and waiver of liability shall serve as a release, discharge, and acceptance of risk for my heirs, estate, executor, administrator, and assigns all members of the vehicle owner/operator’s family.
UNDRIVEABLE
Yes--In case my vehicle becomes inoperable, unsafe or unlawful for any reason including but not limited to: engine or mechanical failure, security issues, ignition or gas cap key problem, electronic issues, no spare tires, I authorize Central Idaho River Shuttles to exercise the option on my behalf that I have stated on the form, or to use common sense.
EMERGENCY CONTACT
Yes--Should there be a major problem with my vehicle, I authorize Central Idaho River Shuttles to contact my emergency contact number if there is one provided.
Yes--It is in my best interest to notify the emergency contact about my float plan should there be any questions that Central Idaho River Shuttles may need to know.
ADDITIONAL EXPENSES
Yes-- I authorize to pay for all reasonable expenses upon receiving my vehicle.
Yes--In addition, if there are any extra fees, such as gas/oil, tires, or miscellaneous costs I agree to be responsible for this, and will leave clear and exact instructions on how that is handled.
Yes--We will make every possible effort to provide for the safety of your vehicle during the shuttle; however we cannot assume legal responsibility for any vehicle or personal articles including fire, theft or vandalism that may occur while vehicle is parked at a parking lot area or landing, any accidents or for time or cost arising from any indisposition.
CONFIRMATION/CANCELLATION
Yes--I understand that it is my responsibility to read my confirmation e-mail. We will pick up and deliver vehicles according to your instructions and answers.
Yes--Should I cancel, I will do so in the form of written confirmation, via email or mail or in person at least 24 hours before my launch date.
